Located in Jacksonville, Illinois, Elm City Center serves residents of Morgan, Pike, Cass, Green, Sangamon, and Scott counties. Schools, social service and government agencies, families, employers, and other interested persons can all refer people to Elm City Center. Referrals should be made to the Center’s Admissions and Program Advisory Council (APAC).
Basic Eligibility Requirements for all programs
- Be at least 18 years old.
- Have a documented vocational, emotional, mental, or physical disability.
- Be free of contagious disease except those protected by law.
- Be free of behavior which is harmful to them, others, and staff/property.
- Be able to benefit from a program of services.
- Be approved by the Elm City’s Admissions and Program Advisory Council (APAC).
- Be capable of maintaining self in the community with limited assistance.
- Be able to understand and observe safety regulations.
- Be able to accept and maintain acceptable standards of worker grooming and personal hygiene.
Funding Sources
All programs are available regardless of an individuals ability to pay. However, a funding source must be identified to cover the real costs that exist for each program. Funding can come from several areas:
- Self Pay by any source: insurance, personal finances, other sources.
- Illinois DHS Division of Developmental Disability, Illinois DHS Division of Mental Health, Illinois DHS Division of Rehabilitation, Health and Family Services.
- Managed Care Organization.
Additional Eligibility Requirements for Day Programs:
- Funding approval generally comes through an Independent Services Coordinator.
- Completion of the DHS DDD Discovery Tool process completed by a local ISC group.
- Have an identified goal that outlines some skill or outcome a person wants to gain while attending our programs.
- Taking part in production work is not a requirement.
- Maintain an attendance rate of at least 85%.
- Be able to work in groups.
- Willing to put forth the effort to get along with other people and work on projects.
Additional Eligibility Requirements for Residential/Case Management Programs
SUPPORTED LIVING
- Be at least 18 years of age or older and have an emotional disability.
- Meet the requirements of Morgan County Housing Authority and pass a background check.
- Be free of addiction to alcohol and non-prescription drugs.
- Not be under warrant for criminal action.
- Be compatible with co-residents, be able to complete all daily living skills without staff supervision, and be self-medicating.
- Either be working or attending day programming.
- Have a previous history of success in other types of residential programming.
GROUP HOME
same as Supported Living plus:
- Have beginning knowledge of community living skills.
- Be motivated to learn independent living tasks.
CILA HOMES
- Be at least 18 years of age or older and have at least a developmental disability.
- Meet the requirements of DHS DDD CILA regulations, coordinated through an ISC, approved by the DDD Rates process
- Pass a background check.
- Be free of addiction to alcohol and non-prescription drugs.
- Not be under warrant for criminal action.
- Be compatible with co-residents, be able to complete all daily living skills without staff supervision,
- Medication management assistance is available or someone could be on a self medicating plan.
- Either be working or attending day programming.
SUPPORT SERVICES
(most services provided to people living independently in the community)
- Be 18 years or older.
- Have a prior or continuing emotional disability.
- Each individual program may have additional eligibility requirements as determined by the funding source, contractual agreements, and availability of funds, and openings in the programs
