These services are available in the Adult Waiver Residential services are available, based on need, to participants who request the service and meet the priority population criteria for residential services.\u00a0 Residential means individually tailored supports that assist with the acquisition, retention, or improvement in skills related to living in the community.\u00a0 These supports include:<\/p>\n
Services may include necessary nursing assessment, direction and monitoring by a registered nurse, and support services and assistance by a registered nurse or a licensed practical nurse to ensure the participant’s health and welfare.
\nServices may also include non-medical transportation between the residence and other community locations, including stores, recreational and socialization activities, and other services.<\/p>\n
A living arrangement provided to Adult Waiver participants in a group home, family home or apartment where eight or fewer unrelated adults with developmental disabilities reside under supervision of the community developmental services agency. Residents receive a comprehensive individualized array of residential options, personal support services and supports under the direction of a community support team within the local agency. Funding comes through DHS Division of Developmental Services.<\/p>\n
These homes have staff on-site at all times Individuals are present. Staff provide both scheduled and unscheduled supports and services as needed by Individuals.<\/p>\n
Serves Individuals whose service plan documents that they do not require 24-hour staff presence, and documents the situations in which individuals may safely be away from staff direct supervision. Intermittent Residential services in apartments or family homes have staff available on call 24 hours per day.\u00a0 On-site staff are available to provide both scheduled and unscheduled supports and services as needed by the Individuals served and as specified in each Individual’s service plan.<\/p>\n
At Home Day Programs are part of the residential\u00a0 program for Adult Waiver participants.\u00a0 At Home Day Programs are based in the individual’s CILA home and provide a structured personalized program of in-home and community\u00a0 activities for Individuals who are unable to participate in out-of-home day programs because the Individual:<\/p>\n
A Group Home is a living arrangement provided to participants in a group home, family home or apartment where eight or fewer unrelated adults with mental illness and \/or developmental disabilities reside under supervision of the community services agency.\u00a0 Residents receive a comprehensive individualized array of residential habilitation, personal support services and supports under the direction of a community support team within the local agency.\u00a0 Funding usually comes through DHS Division of Mental Health.<\/p>\n
A living arrangement where the person has their own relationship with a landlord and is basically living independently.\u00a0 Some supervision can be provided if the person wishes the assistance. Case management and case coordination services are always available on a as needed basis.<\/p>\n
A living arrangement where the Individual owns their own home and is living independently on their own or with family.\u00a0 Some supervision can be provided if the person wishes the assistance.\u00a0 Case management and case coordination services are always available on an as-needed basis.<\/p>\n
A staff member is available 24 hours a day to assist tasks as needed, help plan for recreational activities, and to find solutions to problems as they arise.<\/p>\n
Each Individual lives in a setting appropriate to their ability. Some Individuals need little assistance, while others will learn various household tasks then move to a less structured living arrangement such as supported apartment or independent living.
