The level of support can range from simple assistance to balance a check book to complete 100% assistance for all personal needs.<\/p>\n<h4>Activities carried out in collaboration with the persons served to achieve goals for independence as defined by the persons served.<\/h4>\n<ul>\n<li>Self-advocacy, self-help groups, and assertiveness training.<\/li>\n<li>Developing socially appropriate and age-appropriate behaviors.<\/li>\n<li>Interpersonal relationships including those with the person\u2019s spouse, family, and friends.<\/li>\n<li>Life issues and transitions (e. g. , leaving home, substance abuse, parenting, divorce, retirement, and death).<\/li>\n<\/ul>\n<h4>Employment\/income generation activities<\/h4>\n<h4>Optimizing resources and opportunities through:<\/h4>\n<ul>\n<li>Public Assistance, Community linkages\/Access(SSI, SSDI, food stamps, bus passes, public health, and other state\/local services as needed) and other financial needs<\/li>\n<li>Financial and legal management including purchasing, payee representation, banking, handling taxes, budgeting, and repaying debts<\/li>\n<li>Individual affairs and rights<\/li>\n<li>Appropriate linkages when medical, healthcare, psychiatric, alcohol\/drug treatment \u00a0needs of the persons served are identified.<\/li>\n<li>Coordination of, or assistance with, crisis intervention and stabilization services, as appropriate.<\/li>\n<li>Monitoring use of and educating about prescribed medications<\/li>\n<\/ul>\n<h4>Community and Residential Assistance with:<\/h4>\n<ul>\n<li>Mobility, travel and community transportation skills.<\/li>\n<li>Securing safe housing that is reflective of the abilities and preferences of the persons served<\/li>\n<li>A personalized setting for individuals living in ECC residential programs or on their own.<\/li>\n<li>The opportunity for expression of choice by the persons served in regard to rooms and housemates.<\/li>\n<li>Changing residences at request of individual and or family<\/li>\n<li>Community activities.<\/li>\n<li>Cultural activities.<\/li>\n<li>Social activities.<\/li>\n<li>Recreational activities.<\/li>\n<li>Spiritual activities.<\/li>\n<\/ul>\n<h4>Provision of, or linkage to, skill development services needed to enable the person served to perform daily living activities, including, but not limited to:<\/h4>\n<ul>\n<li>Budgeting<\/li>\n<li>Nutrition and diet management<\/li>\n<li>Personal hygiene and basic self-care skills<\/li>\n<li>Daily access to nutritious meals and snacks.<\/li>\n<li>Meal planning, preparation<\/li>\n<li>Shopping<\/li>\n<li>Human sexuality.<\/li>\n<li>Infection control<\/li>\n<li>Safety practices including dealing with injuries and life-threatening situations<\/li>\n<li>Exercise and fitness<\/li>\n<li>Housekeeping and home maintenance skills.<\/li>\n<li>Problem-solving and decision-making skills.<\/li>\n<li>Recreational and leisure time activities.<\/li>\n<li>Use of phone and computer resources.<\/li>\n<li>Utilization of community services and resources (e. g. , Laundromats, the library, post office, church, stores and individual affairs office).<\/li>\n<li>Contingency planning, problem solving, decision making.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Based on the needs of the individuals, case management\/care coordination services provide the supports needed to help people be independent as they can.
